All Hawkeye Care Centers operate within our established compliance program, which assists all staff members in upholding the standards of conduct in their everyday activities.
Our compliance program is directed by a compliance committee and guides all employees with a written policy, procedures and standards of conduct. The program includes training and education, effective communication, enforcing standards through disciplinary guidelines while conducting consistent monitoring and auditing.
Employees and residents’ family members are encouraged to discuss any concerns with a Facility Supervisor or Administrator.
Concerns related to state and federal laws should be reported right away to the Facility Administrator or the Corporate Compliance Office. If preferred, concerns may be reported anonymously. To report a concern anonymously, please complete a simple form by clicking below. A facility code is required to complete the form. Codes are posted near the entrance in each facility.
All concerns submitted anonymously are collected by a third-party vendor to ensure confidentiality. Concerns are reported to the Corporate Compliance Office for review and appropriate action.
